• Original Birth Certificate (with raised seal)

• Proof of Residency (valid NJ driver's license, current tax bill/deed/lease agreement/contract of sale/mortgage bill/utility bill/affidivat)

The following documents need to be submitted to the Main Office by August 15, 2024:

• Physical Form (exam must be completed after September 1, 2023)

• Health History Form

• Immunization Record
