Physicals for Winter Sports Due: November 4th, 2024

First day of tryouts: November 25th, 2024
Participation Fee: $75.00/per sport

To allow adequate time for the clearance process, this form, located in your Genesis Parent Portal, must be completed in its entirety by the deadlines listed above. Physicals are valid for 365 days prior to the first day of practice.
Fall (Cross Country, Soccer, Volleyball): In-Progress
Winter (Boys' Basketball, Girls' Basketball): Paperwork Due: 11/4/24
Spring (Baseball, Boys' Lacrosse, Girls' Lacrosse, Softball): Paperwork Due: 3/10/25
Physical Forms are good for 365 days and must be valid by the 1st day of tryouts.
As a reminder, physicals are reviewed by our school physician for final clearance. You will be able to check the progress of your students' eligibility in your Genesis Parent Portal. Additionally, once your student-athlete is cleared, you will receive a letter confirming his or her clearance to play sports.
Any missing items may result in the delay of your child’s participation in the season. Please make sure that your child’s physical includes written documentation of vision assessment, or a letter from your child’s eye doctor that indicates visual acuity. Per the state law, our school physician will deny clearance to any student who does not have this.
